From local governments, to hospitals and clinics, and community-based organizations, we work together to make health care happen.
The results of the Development and Implementation of a Navigator-Facilitated Care Coordination Algorithm (NAVCOM) to Improve Clinical Outcomes of Underserved Primarily Latino Patients with Uncontrolled Diabetes project were published in the Journal of Health Care for the Poor and Underserved, November 2013.
The Primary Care Coalition of Montgomery County (PCC) is making health care happen. PCC works with clinics, hospitals, health care providers, and other community partners to coordinate health services for our most vulnerable neighbors.
