Part-Time Community Psychiatrist

Bilingual RN Case Manager

The Montgomery County Cancer Crusade Program, a cancer screening, prevention, education and screening program, is seeking an experienced and dedicated team member to serve as a clinical case manager. 

The RN Case Manager will be responsible for providing case management and patient navigation for patients referred for colorectal and prostate cancer screening.  The RN Case Manager will provide patient medical assessments. The RN Case Manager will support participating Montgomery Cancer Crusade and National Cancer Institute medical providers to determine which medical providers’ cases will be referred. 

Schedules appointments for screening, diagnostic, and or treatment procedures (diagnostic testing, biopsy, surgical or oncology) for patients with abnormal screening results or cancer.  Responsibilities also include timely referrals and follow-up with eligible low-income and uninsured patients in the program, ensuring that each client is given the standard quality of clinical care treatment and coordination. 

The candidate will work collaboratively with clinic medical directors, private physicians, surgeons, hospitals and others to ensure effective patient treatment.

Bilingual English/Spanish Case Manager RN or LCSW/LGSW

The Primary Care Coalition (PCC), located in Silver Spring, MD, is seeking an experienced and dedicated team member.  PCC manages the Care for Kids Program that provides primary health care services, dental care, optometry, and specialty services to children of low-income families who cannot afford private health insurance and are not eligible for federal and state programs.  Care for Kids has over 3,000 children enrolled.

RN-Specialty Care Referral Coordinator

The Primary Care Coalition (PCC), located in Silver Spring, MD, is seeking an experienced and dedicated contract team member. The Specialty Care Access Program assists adults enrolled in the Montgomery Cares program with access to specialty care services through Project Access, a network of private physicians who contribute to the health care “safety net” by donating their services to patients.  The Primary Care Coalition maintains the Specialty Care Access Program through its Center for Health Care Access.  To learn more about PCC please visit our website: www.primarycarecoalition.org.

Americorps Project CHANGE

The Primary Care Coalition is seeking an individual to fill and AmeriCorps-sponsored position.  This position will provide administration and coordination for the Center for Health Improvement (CHI) and specific CHI projects, including:  Cancer Prevention,k Community Engagement, and Quality Improvement.

AmeriCorps, often called America's "Domestic Peace Corps," started operations in 1993.  Project CHANGE is seekign members for its 2010-2011 service year, which runs from September 2010 to Summer 2011.  Members receive a living stipend of $15,000, and educational award of $5,300, health benefits, training and a life-enriching experience.

Manager of Grants Development

The Primary Care Coalition (PCC) is a regionally recognized leader in ensuring health care access and quality for low-income individuals, and reducing health disparities among diverse populations.  The PCC has a $13M+ annual budget of which ~$3M is grant revenue, mainly from regional foundation partners.  As the PCC has grown in size and reputation, so has the potential interest of funders regionally, nationally and at the Federal level. 

The PCC seeks a committed grants professional who will grow our internal grants development process, develop strategies to attract, cultivate and retain major sources of grant funding and build and manage our relationships with regional and national foundations.  In addition, the PCC is seeking continued expansion through Federal grant support.  Prior Federal grant experience is desirable.
