The Primary Care Coalition works with a variety of partners and collaborators to improve access to high quality care for the most vulnerable residents of our community. From local governments, to hospitals and clinics, and community-based organizations, we work together to make health care happen.

The Primary Care Coalition (PCC) is making health care happen. PCC works with clinics, hospitals, health care providers, and other community partners to coordinate health services for our most vulnerable neighbors. The PCC is a 501(c)(3) nonprofit organization.
