“At the heart of the SNF Alliance is a shared vision: seamless, high‑quality care across the continuum,” explains Juhi Morrison, director of clinical programs at the Primary Care Coalition (PCC). “Transitions that support patients and outcomes that improve because we are aligned, intentional, and accountable to one another.”
Partners from skilled nursing facilities and hospital transition teams show up consistently for the complex, challenging work of this longest-running Nexus Montgomery program. The Nexus Montgomery Regional Partnership is a collaboration among all six Montgomery County hospitals to jointly invest in community health and create impact no single hospital could achieve on its own. As the partnership’s management entity, PCC supports partner alignment and moves shared priorities from intention to action.
For nearly a decade, the SNF Alliance has successfully leveraged quality improvement collaboration to decrease rehospitalization from SNFs and create better patient outcomes. Partners—including 37 participating SNFs—review data, coordinate care, and problem-solve around patient care challenges.
“This work doesn’t happen by chance,” Morrison notes. “It happens because busy teams make time, because leaders are willing to share insights and adapt, and because there’s a shared belief that we are stronger together than working in silos.” That commitment has led to real progress—stronger communication, more effective use of data, deeper relationships, and improved patient outcomes. The May gathering celebrated both process and outcomes with recognition awards for partner teams. Among the categories presented were:
- Outstanding Engagement Award to recognize facilities that showed exceptional commitment through consistent leadership and full participation across all SNF Alliance meetings throughout the year.
- Technology Adoption Award to recognize a facility embracing innovation and effectively leveraging tools such as Real Time, CRISP, and PCC to strengthen care coordination and proactively support patient needs.
- Collaboration Champion Award to recognize strong coordination, communication, and commitment to improving patient outcomes together.
- Most Improved SNF Award to celebrate facilities that embraced change, leaned into the data, and made meaningful strides in improving outcomes for their residents over the course of the fiscal year.
“What’s most powerful about this alliance is not just what we’ve accomplished, but how we’ve done it—with trust, respect, and a genuine spirit of partnership,” says Mindy Pierce, PCC’s senior director of population health. “As we celebrate, we also look ahead. The need for strong, integrated partnerships like this one has never been greater, and the foundation we’ve built positions this alliance to continue leading, innovating, and setting a standard for what collaboration across the continuum can look like.”
