“Data, information, and the analysis of that information are essential to modern healthcare,” says Fareed Anjum, PCC’s Director of Data and Informatics. “Everything that happens in a healthcare facility is supported by information—not only data specific to each patient, but also aggregated data that helps establish standards and benchmarks for overall clinic performance.”
Seven of PCC's health center partners use the eClinicalWorks (eCW) platform. Originally designed to digitize healthcare workflows and communication, eCW has evolved into a unified, cloud-based solution that helps improve quality of care, reduce errors, control costs, and deepen clinical insight. According to Fareed, “IT platforms in general—and eCW in particular—make holistic care possible. By collecting, organizing, and analyzing patient data across the entire continuum of care, from check-in through post-visit follow-up, providers are better equipped to deliver comprehensive treatment. You simply couldn’t operate an efficient, modern healthcare facility without it.”
True to PCC’s mission, the organization brings its own vision and values to the technology it uses. “We’ve been using eCW across multiple health centers for more than ten years,” Fareed explains. “That longevity has allowed us not only to understand the platform’s full capabilities, but also to tailor it to the specific needs of the communities we serve.”
One example of this tailored approach is PCC’s focus on reporting and analytics related to the most prevalent health challenges in its patient population, including diabetes, hypertension, cancer prevention, behavioral health screening, and cardiac care. PCC’s data team generates Clinical Quality Measure reports for more than 15 topics on a quarterly and an annual basis. These reports closely follow HEDIS Medicaid benchmarks and offer deep insight into clinical performance and the overall patient health. By prioritizing these areas, PCC ensures that its data efforts directly support better outcomes for the community.
“We don’t just invest time and resources in developing detailed reports and analytics to improve diagnostic accuracy and post-visit care,” Fareed explains. “We also provide hands-on training to help system users fully utilize built-in capabilities such as the Clinical Decision Support System (CDSS). These reminders and alerts reduce the risk of missing critical care needs and ensure a more consistent, accurate standard of care. When clinicians are supported this way, they can offer better treatment options and monitor recovery more confidently.”
Much of this work remains invisible to patients—but it is essential. Like blood flowing through the body, a constant stream of data pulses through partner health centers and PCC every day, sustaining the system from within.
