Every year the PCC monitors the quality of care provided by safety-net clinics operating in Montgomery County and implements grant-funded programs and initiatives that raise the standard of care. From improving cancer screening rates to establishing patient centered medical homes and preparing clinics to become Medicaid providers, the PCC provides technical assistance to help our partners apply best practices at their clinic sites.
The PCC has developed a method of working with community partners to build primary care collaboratives and share best practices. Drawing on this experience, PCC developed a guide to share recommended interventions and successful strategies. The approach described in the Guide for Quality Improvement in a Safety Net Setting can be used by any primary care practice interested in improving its care.
