Sepsis is the leading cause of rehospitalizations from Skilled Nursing Facilities [SNFs] to the Nexus Montgomery hospitals. As of January 2021, the SNF Alliance is now a CTI looking to impact total cost of care in the 60-days between hospital discharge to SNF placement. The broader strategy for this will require a more holistic approach to the key drivers which are: hospital utilization, SNF length of stay, and rehospitalization (SNF to hospital). For this, Nexus Montgomery partnered with The Grant Group to develop a two-day train-the-trainer training session on Sepsis. The goal of the training is to reduce rehospitalizations and readmission, improve quality of care and enhance the collaborative relationship between the SNFs and Nexus hospitals.
