Through data collection and analysis the PCC measures population health, identifies trends, and supports local health planning and improvement activities. Data allows us to see trends in the health of the population and to analyze whether health improvement initiatives are truly working.
The PCC has a dedicated health informatics team working on a variety of projects to improve access to information and data capture and analysis. The PCC administers a shared electronic medical record for eight safety-net clinics, and receives and aggregates data form other sources to conduct powerful population health analysis.
