Kaiser Permanente is offering a chance to receive free, tailored technical assistance to support virtual care delivery, and support from your peer organizations in the DMV region and across the country. KP is bringing together a community of innovators to redesign care so that telehealth continues after the COVID-19 pandemic abates and beyond. This program is designed for FQHC, FQHC look-a-likes, and organizations serving patients experiencing homelessness.

Program Details
Applied Contrat Track
Are you working on a project, or have one in mind, that improves virtual care delivery for a population facing barriers to accessing care? Grant funding opportunities available up to $50,000.
What to Expect:
What to Expect:
- Leverage design thinking and quality improvement tools to improve virtual
care delivery for a specific population facing barriers to accessing care, such as limited English proficiency and low technology literacy. - Learn from experts from the field, including Kaiser Permanente.
- Collaborate with your peers and CCI coaches to accelerate your project.
- Access personalized support and resources that will help you test and implement your project.
- Create a core project team (4-6 people) that can work to advance a specific project over the 15-month period.
- Have at least 1 team member who can participate in program activities (i.e., content webinars, and virtual site visits.)
Clinic Connection Track
Are you rapidly deploying virtual care, but need space to brainstorm creative solutions for the specific challenges you’re experiencing? Grant funding opportunities available up to $12,000.
What to Expect:

- Collaborate with organization across the country to solve common virtual care challenges.
- Learn from experts from the field, including Kaiser Permanente.
- Brainstorm creative solutions to outstanding challenges like improving population management and provider burnout.
- Access robust resources on virtual care best practices.
- You have 1 team member that can participate in program activities but looking for a lighter touch learning program.
Learning hub - open to all
The Learning Hub is open to all safety net health care organizations or primary care organizations who serve people experiencing homelessness, or individuals affiliated with these types of organizations. Hub members have access to:
- Online courses
- Interactive forums for peer learning
- Other online resources.
