This 15-month program will enable safety net provider organizations to learn from peers and experts, test new approaches, accelerate the work they have already started, and develop approaches to sustain virtual care as an essential component for how care is delivered into the future.
There are three types of programs to apply to -- two grant-funded opportunities and a free learning hub. Details for each are found below.
What to Expect:
- Leverage design thinking and quality improvement tools to improve virtual
care delivery for a specific population facing barriers to accessing care, such as limited English proficiency and low technology literacy. - Learn from experts from the field, including Kaiser Permanente.
- Collaborate with your peers and CCI coaches to accelerate your project.
- Access personalized support and resources that will help you test and implement your project.
- Create a core project team (4-6 people) that can work to advance a specific project over the 15-month period.
- Have at least 1 team member who can participate in program activities (i.e., content webinars, and virtual site visits.)
What to Expect:
- Collaborate with organization across the country to solve common virtual care challenges.
- Learn from experts from the field, including Kaiser Permanente.
- Brainstorm creative solutions to outstanding challenges like improving population management and provider burnout.
- Access robust resources on virtual care best practices.
- You have 1 team member that can participate in program activities but looking for a lighter touch learning program.
- Online courses
