“This is an exciting time for the PCC and the community we serve; a time of growth and opportunity. We are excited to have Avi and Jeff join our team as we continue to shape our programs and expand our activities to address the convergence of population health, health care utilization and individual activation for improved health and experience of health care” said Ms. Graham.
The PCC works on many levels to increase access to health care and improve the health and lives of low-income, culturally diverse residents of Montgomery County, Maryland and the surrounding communities. As members of the executive team, Mr. Fichman and Mr. Goldman will work with diverse stakeholders to develop systems of care that meet the needs of vulnerable community members and respond to the challenges of today’s rapidly changing health care industry.
